In order to qualify for home care, patients must:

Be under the care of a licensed physician willing to order home health services

Have a medical need for the services offered by Orthopaedic Home Health and Rehabilitation, Inc.

Be willing to assume an active role in the planning and delivery of care, or have a caregiver willing to assume this responsibility.

Live within the boundaries of Palm Beach County.

Be homebound as defined by the Centers for Medicare and Medicaid Services (CMS) if billing Medicare. Medicare considers a patient to be homebound if they:

Have an illness of injury which restricts the ability to leave home except with a walking aid, special transportation or another person and/or
-
Have a condition where leaving the home is medically contraindicated and/or
-
Are absent from the home only for short, infrequent periods, or to receive medical care
Medicare pays 100% of home health charges.
